Approved Carrier Requirements

1. Signed Broker‐Carrier Agreement (attached via DocuSign)

2. Completed W‐9 ‐ (an optional blank W‐9 has been included for your convenience) 

3. Photo of power unit(s) and trailer(s) that will be used for transport to onboarding@safelanetrucking.com.  

*Images must clearly displays the company logo and DOT information.  

*The information must match details found on the FMCSA database.  

4. Certificate of Insurance emailed to Onboarding@SafeLaneTrucking.com or faxed to 210-817-7084 with:

5. Insurance Requirements

Cargo Insurance Minimum:

$50,000 for 1 unit trailer

$150,000 for 3 unit trailers

$250,000 for 5+ unit trailers ($50,000/unit)

Liability Minimum:

$1,000,000
